Why the latest revision may still be wrong.
A document can be the newest issued revision and still be inconsistent with the current engineering state of the project.
THE DISTINCTION
Latest revision is not the same as current engineering truth.
Revision numbering tells us which document was issued most recently. It does not prove that all dependencies affected by later engineering decisions have been updated.
EXAMPLE
One valve change. Several stale deliverables.
A control valve may be replaced and its datasheet updated, while the loop drawing, hook-up drawing, I/O assignment, wiring or BOM still describe the previous configuration.
MUR PRINCIPLE
Track engineering state, not only document state.
MUR treats an approved engineering change as the starting point. Affected dependencies are identified, relevant deliverables are marked stale, and only then are they regenerated and validated.
